Appointments
It is our policy to see patients by appointment, except in cases of emergency.
Appointments are scheduled Monday through Friday between the hours of 8:30 a.m. and 3:30 p.m. If you are unable to keep your appointment, please notify our office.
If you are having an urgent problem, please inform the receptionist and an appointment will be made as soon as possible.
Every effort is made to see patients at their appointed time. Occasionally, surgeries last longer than anticipated and deliveries are unpredictable, which may result in an unexpected delay in your appointment. We appreciate your patience when these situations arise.
Fees
An estimate of charges for any visit or surgical procedure will be given upon request.
Payment, including co-pays, is required for all office services at the time rendered.
If you have questions regarding your account, please call our Insurance/Bookkeeping Department at 256-533-7423.
All accounts which remain outstanding without explanation may be forwarded to the Credit Bureau for collection.
Prescription Refills
If you need a prescription refilled, please call your Pharmacist first. Their records will indicate if you have an authorized refill.
If there are no refills, then please call our office during office hours. If the prescription can be refilled, we will call your pharmacy. We do not call in routine refills after office hours.
Telephone Calls
Our Telephone Office Hours are Monday – Friday, 8:15 a.m. and 4:15 p.m.
Please call our office during our Telephone Office Hours if you have any questions regarding your condition, medication or treatment. We are aware that any matter concerning your medical care is confidential; however, the phone receptionists need some specific information in order to properly route your call. Our medical staff is trained to answer questions, arrange prescription refills, and schedule appropriate appointments.
Except in emergencies, the doctors do not leave their patients to answer phone calls. If you need to speak with the doctor personally, you may leave a message and your call will be returned when conditions permit, usually at the end of the day.
If you have an URGENT problem after our Telephone Office Hours, our answering service will take your phone number and a nurse or physician will return your call as soon as possible. For all non-urgent issues, please call during our Telephone Office Hours.
In a TRUE EMERGENCY, it is best to go to the hospital Emergency Room. The physician on duty will begin treatment and contact our on-call physician.

Insurance
Our Physicians participate in the following health insurance plans:
Aetna, Blue Cross Preferred Care, First Community, First Health, Health Partners, Medicare, NAMCI/Premier, One Health, PHCS, Prime Health, United Health, and Viva.
You must bring your insurance card with you to your visit. You are responsible for your co-pay, deductible and non-covered charges. Please remember your insurance contract is between you and your insurance carrier. Therefore, you should direct any questions or complaints regarding your coverage to your insurance carrier.
If we do not participate in your insurance plan, you will receive an itemized statement at the end of your visit that you may use to file with your personal insurance. However, PAYMENT IS DUE AT THE TIME SERVICES ARE RENDERED.
Surgery and delivery charges will be filed by our office. After insurance payment has been processed, you will be billed for any unpaid balance.
